Position Summary
The Dietitian provides comprehensive nutritional care for participants enrolled in the PACE (Program of All-Inclusive Care for the Elderly) program. Reporting to the PACE Program Manager, the Dietitian assesses nutritional needs, develops and implements care plans, and evaluates outcomes. The Dietitian works collaboratively with the interdisciplinary team to ensure participants receive high-quality, safe, and effective nutrition services while maintaining professional competency and adhering to applicable laws and regulations.
Key Responsibilities
Assess participant nutritional and medical histories, laboratory results, current diet orders, medications, and nutritional regimens to determine estimated nutritional needs.
Develop and implement individualized nutrition care plans to maintain or improve health outcomes related to nutritional diagnoses.
Conduct nutrition assessments in accordance with regulatory guidelines to support participant care planning.
Educate participants, families, staff, and contracted providers on diet, nutrition, food safety, and sanitation.
Document relevant participant information in the electronic medical record (EMR) and update the participant plan of care.
Participate in daily Interdisciplinary Team meetings and program activities as scheduled.
Maintain knowledge of PACE Federal, State, and local regulations, organizational policies, and ethical standards.
Perform additional duties as assigned to support program objectives.
Qualifications
Education:
Bachelor of Science in Dietetics, Food, and Nutrition or equivalent meeting the American Dietetic Association requirements.
Completion of an accredited internship or coordinated undergraduate program as per ADA guidelines.
Licensure / Certification:
Registered Dietitian (RD) or eligible with the Commission on Dietetic Registration.
Licensed or license-eligible in the State of Iowa.
Current BCLS/CPR certification.
Adult Mandatory Reporter status.
ANSI-accredited food protection/safety certification (e.g., ServSafe Food Protection Manager) or obtain within 90 days of hire.
Valid driver's license and auto insurance.
Skills and Abilities:
Effective oral and written communication to interact with participants, families, and healthcare professionals.
Ability to work collaboratively within an interdisciplinary team.
Strong organizational skills and ability to manage multiple priorities.
Proficiency in Microsoft Office products.
